PACCT is an online application (https://pacct-adhsgis.hub.arcgis.com) for Arizona post-acute care facilities to input their public health surveillance and bed availability data as required by Executive Order 2021-19, Enhanced Surveillance Advisory. These facilities are required to report information specified in the Enhanced Surveillance Advisory to ADHS every twenty-four hours. All persons required to report pursuant to an Enhanced Surveillance Advisory must cooperate with ADHS and local health authorities in effecting the Enhanced Surveillance Advisory. Failing to to report this information is an act of unprofessional conduct.

This required reporting allows ADHS and other public health and emergency preparedness entities to maintain awareness of post-acute facility capacity for COVID-19 patients. This reporting also allows acute care hospitals the ability to easily see bed availability and place discharged patients with COVID-19 accordingly. PACCT has two user types and depending on the user's assignment, a user will have access to different areas of the application. Post-Acute Care Providers will input information via a survey application (Survey Users); whereas Acute Care Hospitals, emergency preparedness and county public health users will have access to view selected information provided in the surveys via a data dashboard (Dashboard Users). Those users with access to view the data dashboard will have no editing capabilities.

The Arizona Post Acute Care Capacity Tracker (PACCT) online application has been developed in conjunction with the statewide plan for information technology as set forth in A.R.S. § 41- 3504 (A) (1). It is a component of the Arizona Department of Health Services (ADHS) Information Technology Services, which may be accessed and used only for official business by authorized personnel. Unauthorized access or use may subject violators to criminal, civil, and/or administrative action. As a State-owned system, there is no right to privacy on this system. All information on this system may be monitored, intercepted, recorded, read, copied, and shared by authorized personnel for official purposes including criminal investigations.

User Agreement

Terms of Agreement:

The terms of this Agreement shall become effective upon signature and shall remain in effect for three years after the date of signature, unless restricted or terminated earlier. An Arizona Post Acute Care Capacity Tracker user will be required to renew the Post Acute Care Capacity Tracker User Agreement on a three-year basis. ADHS may, in its sole discretion, terminate a user account at any time, including in response to any unauthorised or inappropriate use.


Security Requirements on the Post-Acute Care Capacity Tracker Application

I, hereby attest in submitting this form that the facility identified meets and has established the criteria as outlined in this user agreement including that the facility has and will maintain the following: *

User will need to change the password and set a security question upon initial login.
User will be required to renew the PACCT User Agreement on a three-year basis.
User will be limited to five (5) login attempts before losing access for 15 minutes.
If a password is forgotten, it can be reset using the security question set at initial login.
User will notify ADHS PACCT Support Team at pacct@azdhs.gov within 24 hours of any unauthorized use or release of information obtained through PACCT.
User will not leave their computer unattended when logged into PACCT.
User will not share PACCT information for any purposes other than the official use outlined in this Agreement.


Agreement Provisions

ADHS has a duty pursuant to A.R.S. § 18-522 Anti-identification Procedures to develop and establish commercially reasonable procedures to ensure the security of entity identifying information. In consideration of the Department's duty and my responsibilities as a PACCT user, and in recognition of the potential harm or discomfort that could be caused by the release of sensitive, provisional, and/or facility information obtained while using PACCT, I agree to the following provisions:

Only the following authorized use will be allowed: Survey Users: Will only access and view the PACCT survey in order to input the required reporting information. Users have a responsibility to ensure the accuracy of the information inputted into this application. Users may be given the capacity to download their own facility's data at a later date. Dashboard Users: Will only use the PACCT dashboard information for patient placement from their own healthcare system or for public health surveillance. The dashboard information is to be viewed by authorized users only and shall not be made publically available.
Not to share my PACCT information (including user ID, password, or facility information) with anyone not authorized to enter this information or allow unauthorized users (anyone outside my organization or unauthorized individuals within my organization) to view the PACCT Dashboard or any other PACCT data (including information inputted into a survey).
To require any and all facility employees or agents that receive or have been given proper access to the PACCT to agree to the same terms and conditions included in this Agreement.
To use all information viewed or accessed on the PACCT solely for the purposes of public health, patient placement or emergency preparedness and not for personal or commercial gain.
To not share or otherwise disclose any PACCT information without the explicit consent of ADHS.
To request consent, email pacct@azdhs.gov with what information you want to share, with whom you want to share it, and via what platform you want to send, share or distribute the information. ADHS will evaluate your request and in no way is obligated to give consent.
Unauthorized release of PACCT information may result in immediate termination of access and notification will be sent to the user's facility administrator and/or supervisor.
To report any use or disclosure of PACCT information of which it becomes aware that is not permitted by this Agreement.
If it is discovered the use or disclosure of PACCT information is not reported, ADHS reserves the right to limit or revoke access for violation(s) of this provision.
To not override or circumvent the security procedures related to PACCT. This includes not introducing any harmful technology, or otherwise attempting to interfere with the proper working of PACCT, or any portion thereof.

I have reviewed and understand the above Agreement and agree to be bound by both with regards to my access and use of PACCT. This Agreement constitutes the agreement between myself and ADHS regarding the use of PACCT. By accessing PACCT, I acknowledge that I have read and understand the above Agreement, and I agree to be bound by the above terms and conditions.
